Don’t Forget Your New Patient Forms

We’d like to think our reception room is incredibly comfortable, but we know we don’t compete with your couch, so take a moment to fill out these forms before you come in for your appointment. This will also help us minimize your time spent in our reception room!

During your first visit, we’ll need to obtain important information, like your medical history and background. Please fill out & print the following forms so we maximize your time with Dr. King.
